- [Voiceover] Our responsibility is to do no harm
and to protect the patients.
- Well, we started to see a trend
in central line insertion complications.
Along with that, we were also seeing
some increases in central line infections.
- Patients were staying longer in the ICU.
They were having to have multiple tries at access.
- There's an occasional patient that was dying,
and dying with diseases that should not
have lead to death, and we as a community
of doctors here decided that we'd
put an end to that, and our goals
were not directed towards just reducing the rates,
but reducing them to zero.
5% of the people that walk into a hospital
in the United States develop some sort
of hospital-acquired infection.
That 5% translates into a lot of deaths.
It translations into a lot of disability
and a lot of cost.
- But those percentage points,
even going up or going down,
is somebody's mom, somebody's relative,
somebody's grandparent, somebody's child.
So we've got to get it right.
- I did a series of debriefings
with our internal medicine and our family medicine
residents, and they actually were the ones
who brought up their concern
with our central line placement,
and they felt like they didn't have
the knowledge and the skills to do it safely,
and so we took that information,
then we started looking at our numbers
and started talking with the rest
of the doctors and really putting together
the big picture of what it meant
to place central lines in east LA.
- Our goal was to standardize the procedure.
We make sure everybody is following
the five steps in the IHI bundle,
and then the sixth step, which is to use
the ultrasound for vein location.
Sonosite was instrumental in helping us
develop that program.
- Sonosite was a great partner
because not only did they help us
with the equipment side of it,
but they also helped us make the IHI bundle real
for our physicians and helped us develop
our educational modules, really helped us
identify what measurements we needed to track
so that we'd know whether or not we were
really making a difference, then helped us
analyze our outcomes.
- That's what helped changed the culture,
Sonosite's passion to help us
have the tools and change the culture.
- The nurses are patients' advocates,
and if they see a doctor isn't washing his hands
or her hands, they see that they're not
donning on the gown the right way
and using proper sterile technique,
we have empowered them with the ability
to stop the procedure.
- As chief of staff, he looked at them and said,
"You know what?
"If you stop any doctor from doing something
"that you think is unsafe
"and he or she gives you a hard time,
"you call me, and I will back you."
(optimistic instrumental music)
- In our hospital, we have reduced mechanical
complications to zero.
- In 2012, here at White Memorial in our critical care unit,
we had a zero infection rate for central lines.
The more days here in the ICU,
the more it costs and the more
the hospital may or may not be reimbursed
for that care, so I think what's really been
important for us is having a good
central line program in our hospital
that limits the complications for patients.
- As we meet with our colleagues in other facilities,
often time we share our infection rate,
and I think they're a little jealous of us.
- Well, last year when Leapfrog released
the hospital ratings, we came in at a B.
Not real happy with a B, but I'll take a B.
So this year when they released the hospital ratings,
we had moved from a B to an A,
so I looked into the details behind why,
and a lot of them are right around
those safety indicators, one of them being
our bloodstream infections and those complications
associated with those type of procedures.
I think one of the great things
about having a project like this
is it changes your fundamental belief
about what's acceptable and what's unacceptable.
And now, for us at White Memorial,
central line infections and complications
are not acceptable.
- Anybody and everybody in health care
really has to believe that anything's possible.
You'll never get the outcome that you're expecting
if you never start to try to think
about the possibilities of changing your process.
And change is hard and difficult,
but if you never start, you'll never finish.

- [Voiceover] Ultrasound has literally
transformed my practice.
I can get closer to the diagnosis, which is so important
figuring out what treatment you can do
to keep people active, to keep people doing
what they wanna do, it makes me a better doctor,
and patients are better off because of it.
I'm Kimberly Harmon, I'm a Team Physician
at the University of Washington.
I'm also the Director of the Sports Medicine Fellowship here
and so we train people who have finished their Family
Medicine residencies, to be Sports Medicine doctors.
Down in the athletic training room, on practice days
it can be a slower paced environment,
but it also depends what's coming up.
So if there's an important game or competition
that's coming up, the sort of urgency in terms of
getting somebody back to play, definitely increases.
The more information you can have about a situation
makes it easier to put people back in or to hold them,
so for instance if you know if something is fractured,
versus if it's just bruised, that's a different
set of decision making.
The real, sort of difference maker in terms of
it being such an important piece of equipment
was when we had our star player, who went down
and he had a contusion to his quad.
By Tuesday, the week before our game,
he wasn't any better, and in fact it was unsure
whether he could practice that day.
- I didn't know what we're gonna do, honestly, you know.
It was kind of getting to the point where
it didn't look like he was gonna be able to participate
at a very high level, and it was when we were first
sort of testing out the ultrasound,
and first getting used to that technology,
and Doctor Harmon was like, well let's check it out
in the ultrasound, let's see what we can see.
- I put the ultrasound on him, we saw this big hematoma,
this big collection of blood in the middle of his quad,
his thigh muscle, and we were like,
well there's your problem, and so just,
grabbed a needle, and drained about 85 cc.
- And that day he felt 100% better.
He was able to run around without a limp,
he felt fine, you know it was just a matter of
the traditional methods we were using,
we couldn't get that fluid out of there fast enough.
And I don't know if he could've played
if it wasn't for her stepping in and saying,
hey let's try this out, let's try this technology out.
- This was really a difference maker
in terms of getting this player back to play.
I think that it's going to become
an integral part of the way we practice
Musculoskeletal Medicine and Sports Medicine.
It's made a huge impact in my practice, my patients'
satisfaction, left less people out for advanced imaging.
- [Rob] There's no reason why every university
in the entire country shouldn't have
ultrasound in their training room.
It's affordable, it's extremely useful,
it allows us to do our job better.
I can't imagine going back now.
Having access to diagnostic ultrasound
in the training room for the past two years,
I can't imagine trying to go back to work without one.
- Medicine has maybe gone for more
paternalistic sort of delivery where me
telling you what you have and you saying okay doc,
and going and doing whatever to more of a
partnership in care, and you need to sort of buy-in
to what I'm telling you in terms of
that you're making the correct diagnosis
and you've got the correct treatment plan,
and that you've really got their best interests in mind
and that you move forward with that.
I've never gotten hugged by more patients in all my life.
When people get better, particularly when they're
not expecting to get better, they're really appreciative,
and it's a really good feeling.

- Soon as we turn on the function of the
advanced neo visualization, the image quality stays the same
and we just see greater echos when we insert the needle.
So I haven't noted any decrement to the
image resolution and quality.
In my practice it's really added increased speed.
I'm getting to the target more directly with less time.
Getting in and out, which always makes the patient happier
and certainly it reduces the amount of pain
that might be involved in a procedure.
- So the ability to see the needle going through tissue
makes their clinical procedures safer,
makes it faster, obviously better for the patient.
- It appears maybe two, three, four times brighter.
Seems to have a better echo during the entire procedure.
- Basically, the enhanced visualization technology
is a softer grade, and it's very easy to use.
There's no extra equipment or specialty needles
that have to be part of the procedure.
Hard to close, it works with any needle out there,
and it's basically available at the push of a button.
- The beauty of it is that there really is no setup time.
It actually speeds up the procedure
'cause it's easier to see the needle.
So if anything it's a time saving technique.
I think it really can enhance learning
and becoming competent in performing aspirations
and biopsies and a variety of different
needle localization techniques
in a much quicker, safer fashion.
- There's really no other competitor that has
anything quite like what we're doing.
There's a few technologies out there
that try to help see the needle tip.
The bigger issue I see with those
is that they take a long time to set up,
and they're also expensive.
There's hardware involved.
This was a proprietary software algorithm
that we can upgrade any turbo or S system on the market.
- You get all the visualization that you really need
and you don't need to use any special needles,
you don't need to use a larger needle.
The larger the caliber of the needle,
the more painful and the more potential for complications
so I get great visualization with a 25 gauge needle.
- The reason we're excited is because now we can
for the first time say that stick needle angles
are no longer a problem for ultrasound imaging.
- I think it's one of our biggest
breakthroughs in a long time,
and again it meets the criteria.
It addresses a real problem that our customers
and our clinicians are dealing with every day.
It's inexpensive, it's simple, and it works.
- It is a game changer in the market
of ultrasound guided procedures.
It's the speed, the ease of use,
the reduction of time to completion of the procedure,
the safety features, all of those are
a win-win situation for doctor and patient alike.

a significantly longer learning curve is just not a simple everything in one
package is very hopeful
to not have to turn on or setup he'd issue
equipment is important to increase your accuracy in
it believed you get to choose that you normally wouldn't be able to

for all those reasons it's the stable
I think for any clinician that's doing injections this is a critical
improvements
this is not one of the minor updates to software
that we see with other computer modalities in this is something is a
major change

- I see tragedy every day.
You could say it's a specialty of tragedy.
- People are sort of at death's door.
- You got people yelling and screaming,
and in that type of environment you gotta
tell somebody about your personal history.
About personal things that you don't talk about
with everybody every day.
And to get somebody to talk about those types of things,
because they're very important in treating a patient,
is an art.
- You see the fabric of society.
- And you see people and how
we treat each other in real life.
- My neighbors always askin' me, did ya see anythin' cool?
And I'm like (inhales),
I don't remember, it was just a blur.
- To save that person's life, I don't have time
to call the radiologist who's probably home
at 4 o'clock in the morning,
say can you come and do the ultrasound for me?
- If you could look right through the body
at the organs you're interested,
why the heck would you not do it?
- I'm sure he would really be happy if he can say,
"Dr. Saylor, why don't you do that and save the patient
"and let me know how it turns out in the morning?"
- There's been a lot of moments like that
with ultrasound where I say, wow,
this guy woulda died.
Or, as a physician, you think to yourself,
I almost killed this lady.
- But now you got a patient here tomorrow.
Are you gonna be able to save this person's life tomorrow?
- Just the other day I had a tragic drowning case
that I was involved with.
A 18-month-old that was found
at the bottom of a pool,
and we were able to get a heartbeat back.
The child did die.
- I've gotta be just that much better tomorrow,
because something else is comin' through that door
that I may need it for.
- As a specialty, we get it right about 80% of the time.
We're sorta 80-percenters.
And a lot of people are comfortable
not gettin' to a hundred percent,
and so that drives some emergency physicians crazy
and out of the field.
- As a father and with many other people
in the resuscitation team that were parents,
to see that sorta case,
that was pretty,
that was a big-impact case.
- At the end of the day can you sit back
and say I did everything I could for that person?
- Within minutes I have to then go back
and see a sprained ankle.
- And hopefully I have a good outcome.
(chuckles)
- Is it more art or science?

- On the battlefield, you have to make decisions quickly,
and correctly, and accurately, or people die.
I was in a line unit, so my job was obviously
to take care of patients.
But my bigger job was to advise the commander.
Do we need a helicopter?
Do we really need to evacuate this soldier?
You have traumatic amputations,
you have pneumothorax, you have hemothorax,
you have intraperitoneal fluid,
pericardial tamponade, you name it,
you're gonna see it in Iraq.
So you transition from an area
where you have every piece of equipment
at your beck and call, to Iraq.
You have your hands.
And you have your stethoscope.
And that's it.
Once you get the ultrasound machine,
you can now look at people and say,
do they have evidence of a pneumo, yes or no?
Do they have evidence of blood in their belly?
Do they have fluid around their heart?
And you can answer these questions in seconds.
I'm a little bit passionate,
because, I mean, you think about patients
who come into the ER,
I mean, they're deserving of health care.
No one would debate that.
But take a look at the soldier.
The way they make a living is by protecting our rights.
We take a piece of equipment,
which relatively is inexpensive,
compared to most things we buy,
whether it's in the hospital or for the military,
and it can dramatically change our ability to say
this person really is sick.
This person really does have this injury.
Or this person doesn't need to be evacuated right now,
we can take our time, we can do it safer.
We can put less peoples' lives at risk.
That, to me, is the beauty of ultrasound.
Because who is more deserving than the people who
really give, without any thought to themselves,
so much to us every single day.
And by taking this piece of technology,
we can make it safer for them to do that very dangerous job.
I guess for me, ultrasound really is piece of mind.
They can have so much more information
so that they can be certain that they're providing
the best health care for that soldier.

- We had a 100 year old guy come in
at around midnight last night.
He and his wife have been married 77 years.
Very pleasant, friendly guy,
with a horrible wrist fracture
and as an anesthesiologist,
immediately when you meet someone,
you think what's the anesthetic I'm gonna do for this guy.
As I'm talking to him, I'm thinking
I'm running through the options in my head,
the more I thought you know I think my best bet here is
to give him nothing and I'm just going to block the arm.
I want him as lucid as possible
and I want to be able to wake him up.
We went into the operating room and I looked at the guy
and normally would do an
infraclavicular brachial plexus block
which it goes through all the muscles,
you're close to the lungs,
you're thinking about all those things,
and he was kinda slouching like this,
so I thought, okay this is going to be
a little bit more technically challenging
and I got him on the table and took a look
and he, amazingly enough, and thank god,
he had this beautiful artery that I could see
and gorgeous brachial plexus, I could see all the cords.
I could see it just flooding the lateral cord,
the posterior cord and the lateral cord.
You know, we did this all on the gurney
before we even transferred him over to the OR bed
cause I asked him pre-op if he had any pain
and he looked at me and goes,
not as long as you don't touch it
and I said okay, don't worry about it,
we're not going to touch it,
so I didn't move him at all.
We did the block, I said does it hurt now,
and he looked at me and the orthopedic surgeon was there too
and I was like I paid the guy 100 bucks
and he goes, oh, it feels a whole lot better now,
and I looked at him, I said you just made my day,
that was great.
